Cancellation and Deposit Policy

A $25.00 service deposit is required for all appointments or consultations and deducted from the total amount due for the service.

Regarding this deposit:

• It will not be refunded if cancellation occurs less than 24 hours before the scheduled service date.
• It will be refunded for cancellations made 24 hours or more before the scheduled service date.

Payment Policy

All services are private pay and are not covered by insurance. However, we do accept Care Credit financing.

Weight Loss Programs

A $25.00 weekly injection service fee applies if injections are administered by the provider.

Concierge Service

Concierge service is available for all services upon request. Please inquire for details if interested.

Service Termination

All services can be terminated at any point during the treatment regimen at the provider’s own discretion based upon continuous breach of service contract.
